Complete the request form with the following information:
- Requesting agency name, mailing address, phone number and fax number.
- If you are a former student requesting the transcript for your own use, fill in the form using your name, home address and phone number.
- Student name, date of birth, dates attended, student signature and date. Student social security number is optional.
- Requesting teacher/registrar name, signature and date.
- Name of the school site the student attended.
Transcript requests will be completed in a timely manner in the order they are received. Phone and email requests will not be processed.
