How to Request an Official Transcript from John Muir Charter School
To request an official student transcript from John Muir Charter School:
Fax a transcript request form to Cathy Akana, Muir Administrative Coordinator. The request must include the following information:
Requesting Agency Information:
Requesting agency name, mailing address, phone number, and fax number.
Student Information:
Student name, date of birth, dates attended, student signature and date.
Student social security number is optional.
Requesting Teacher/Registrar information:
Requesting teacher/registrar name, signature and date.
Helpful Information: Name of the John Muir Charter School site the student attended (for example, “Tahoe CCC site”, “Sacramento Conservation Corps”, or “CCEO YouthBuild Gardena”).
Transcript requests will be completed in a timely manner in the order they are received. Verbal (phone call) and email requests will not be processed. Fax requests can be sent to:
